PRIOR TO YOUR FIRST CONSULTATION
Our initial consultation is a history discussion as a telemedicine appointment followed by an in-person exam. New clients, please be sure to request both a "New Patient Consultation" AND an "Exam" appointment when scheduling.

We require the following online forms be completed prior to your appointment. Please complete BOTH the New Patient Form and the Clinical Medical History Form below and have medical records sent to: info@animalhealingarts.net

 Please note that your appointment will not be accepted if both forms are not completed. Our office offers natural supplements, chiropractic, acupuncture, cold laser and massage for healthy animals and those with mild health conditions to maximize longevity and enjoyment of life. We do NOT offer urgent care, conventional treatment or treatment of advanced disease. We recommend emailing us prior to filling out forms if your pet has an illness to determine we offer treatment for that condition.
